Mount Holyoke College Student-Athlete Medical Clearance
The following instructions must be followed by all those interested in trying out for an intercollegiate team at Mount Holyoke College. Forms must be submitted prior to the traditional or non-tradtional season, whichever comes first in the academic year. Deadlines will be announced by coaches.
First Years/New Athletes
- Physical and Medical History Questionnaire - Sent out by the Mount Holyoke Health Center to all first years and due back to them by July 1. NOTE: Students who return to school and have incomplete Health Center materials can be seen on Monday morning of preseason for a fee of $100.

Returning Athletes
- Returning Athlete Medical History Form
- All students who plan on participating in intercollegiate sports again the following year need to have their blood pressure taken in the Athletic Training Room during the third week of April. A late fee of $1.00 will be charged if students do not make it during the designated week. Students who are abroad will have an opportunity to have their blood pressure taken immediately following the “Introduction to Athletic Training” meeting on first day athletes return for preseason.

NOTE: Second season of the year (i.e. lacrosse traditional season in spring or soccer non-traditional season in the spring) athletes who did not participate in the fall season will need to be medically cleared. Appropriate online forms will need to be completed, and a physical from the Health Center will be required unless one has been completed in the last six months. Athletes should call the Health Center at 413-538-2121 to make their own appointment. They should be sure to state that they need a Sports Medical Clearance.
